Alert 00 – 11 Casing Stabbing Basket Incidents
WHAT HAPPENED:
A fatal incident occurred when an employee was folding up the standing platform of the stabbing basket while standing on the platform. He failed to move off of the platform in time, lost his balance and fell. He was trapped between the stabbing platform and stabbing board support beam. In addition, the platform hoisting chain fell on the operating lever of the air winch causing it to jam on and continue to fold up the platform. Another incident occurred when an employee was moving from the stabbing basket to the derrick. The safety belt lanyard caught on the raise/lower control lever causing the stabbing board assembly to move up, trapping his leg.
